Organisational regulations and fees
The organisational regulations are an internal document of the practice, established by its manager. They are not published in full – the act names four items to be made available to patients at the practice and on the website (art. 24 sec. 2): the type of activity and scope of services, the fee for making medical records available, the fee for storing a body, and the rates for services. All four are on this page.
Organisational regulations
The provider's organisational regulations are available for inspection at the practice's reception, where services are provided. At the patient's request we also make them available in another form.
Fees for services
All services are paid – the practice has no contract with the National Health Fund (NFZ). You will find the current rates in the price list. The amounts given there are for information only and do not constitute an offer within the meaning of art. 66 § 1 of the Civil Code.
Access to medical records
The first release of your records in the scope requested is free – the act says so and we have no say in it. We release records to the patient, their statutory representative, or a person they authorise.
For any further release we charge PLN 0.60 per page of a copy or printout, PLN 3.50 for release on a data medium, and PLN 10 per page of an extract – with a ceiling of PLN 10 per release, whatever the number of pages.
These rates are below the maximum permitted by art. 28 of the Polish Act on Patient Rights. In practice: a complete record on a data medium costs PLN 3.50, and the most expensive request possible costs PLN 10.
Storing a patient’s body
Not applicable. The practice provides outpatient dental services only and does not carry out activity to which this point would apply. We state it because the act lists this information among the four made available to patients.
Patient rights and complaints
Patient rights are set out in the Act of 6 November 2008 on Patient Rights and the Patient Ombudsman. Its text is available for inspection at the practice's reception.
Feedback and complaints
Please send any feedback on the services provided directly to the practice – by phone, by e-mail or in person. We try to reply without delay.
Patient Ombudsman
A patient whose rights have been infringed may contact the Patient Ombudsman (Rzecznik Praw Pacjenta). Free helpline: 800 190 590 (in Polish).
Personal data
How we process personal data is described in the privacy policy.
